Your initial appointment will consist of a consultation.At that visit radiographs may need to taken in order to provide diagnosis and treatment options.
Please assist us by providing the following information at the time of your consultation:
Any Referral Slips and any dental X-rays if provided by your dentist. A list of medications you are presently taking. Dental insurance cards or forms.
IMPORTANT: All patients under 18 years of age must be accompanied by a parent or legal guardian.

If your dentist has taken dental x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.

Dr. Venedikian is open Monday from 12:30pm to 6:30pm, Tuesday 10am to 6:30pm, Wednesday 10am to 5:30pm, Thursday 10am to 7pm, Friday Closed, Saturday 10am to 2:30pm. We will schedule your appointment as promptly as possible. We try our best to stay on schedule to minimize your waiting. Please call with any questions or to schedule an appointment.

For Non-Insurance Patients payment is due at the time service is rendered. For Patients with Insurance Co-Payment is due at time service is renedered. For your convenience, we accept American Express, Visa, MasterCard and Discover.
For our patients who have insurance, we will submit the claim to the insurance company. Most insurance companies will respond within four to six weeks. Any remaining balance after your insurance claim has been processed is your responsibility. We will send you a statement. If you have questions regarding your account, please contact us. Many times, a simple telephone call will clear any misunderstandings. Your prompt remittance is appreciated.

We accept a full range of dental insurance plans and we make every effort to maximize your insurance reimbursement for covered procedures. If you have any problems or questions, please ask our staff. They are well informed and up-to-date. So we may assist you in filing your insurance claim, please provide us with your dental cards.
